It will be wrong sometimes. The design goal isn't an assistant that never errs — it's one where you can always see how it got there.
Open the trace. The steps it took are kept with the answer. Nine times in ten the mistake is visible there — it looked at the wrong term, or the wrong month.
Check the row. Answers link to the records behind them. Open one; either the record is wrong or the answer is.
Rate it. The thumbs are read by a person, and the question text comes with it.
The usual causes, in order
The wrong term is active. Most "that number is wrong" is a term boundary. Check which term is active before anything else.
A bare date. "The 14th" gets pinned to the month in play. Say the month.
The record really is wrong. If nobody marked the roll for three weeks, no assistant can tell you who's at risk.
